When a patient or client needs additional support at home, knowing there is a trusted care team to refer them to can make the next step easier.
Hentro Care Connect works with healthcare professionals, retirement communities and other organisations to provide personalised home-based care that supports each person’s individual needs, independence and dignity.
Whether someone is returning home after a hospital stay, requires ongoing support, or their care needs are changing, our team can help families put appropriate care in place.
The transition from professional or clinical care to everyday life at home can be challenging for individuals and their families. Hentro helps bridge that gap.
We begin by understanding the person’s circumstances and care requirements through a complimentary assessment in their home. From there, we develop an individual care plan and identify a suitable carer based on their needs.
Once care begins, Hentro remains involved through ongoing oversight, communication and review.
Our aim is simple: to help people receive the support they need while remaining in the familiarity of their own homes wherever appropriate.

We welcome referrals and relationships with professionals and organisations involved in the health, recovery and wellbeing of the people and families we support.
This includes:
If you are supporting someone who may benefit from care at home, we would be happy to discuss their circumstances with you.
There are many reasons someone may need additional support at home.
Hentro offers both short- and longer-term care arrangements, depending on the individual’s circumstances.
Contact the Hentro team and provide the basic information needed for us to understand the referral.

Our team will discuss their circumstances and arrange a complimentary home assessment.

The assessment considers the person as a whole - including their health and medical requirements, mobility, nutrition, medication, cognitive and emotional needs, home environment and family circumstances.

Following the assessment, an individual care plan is developed and a suitable carer identified according to the person’s requirements.

Once care begins, the Hentro team continues to oversee the care arrangement, with regular communication, care documentation, check-ins and reviews as needs change.

Healthcare does not stop when someone leaves a consulting room, hospital or rehabilitation facility.
For many people, what happens next takes place at home.
Hentro is here to help make that transition easier for individuals, families and the professionals who support them.
TRUSTED CARE. RIGHT AT HOME.
